Choctaw Nation can pay part of your dental bill.
That payment is called a voucher. You do not pay us first and wait for money back — Choctaw Nation pays our office directly.

Only what is left over after the voucher, and after your insurance if you have it.

Three vouchers exist: $1,000, $900, and $800. Pick the one that matches your treatment.
Call Choctaw Patient Relations
Ask for a dental voucher and have them fax it to Broadway Dental in Poteau. Numbers are at the bottom of this page.
Call us to book
Once the voucher reaches us, we schedule your visit and apply the amount to your treatment.
The three amounts
Each one is separate. Using a crown voucher does not use up your $800 general voucher.

Crown
A cap that covers a cracked, broken, or heavily filled tooth.
If I have insurance
Insurance pays first. The voucher then goes toward what is left.

Removable partial denture
A partial plate that replaces a few missing teeth and comes out at night.

Insurance pays first. The voucher then goes toward what is left.

1 every 12 months
General dental treatment
One shared amount you can put toward most other treatment.
What this one covers
You choose how to spend it across eligible treatment, such as:
- Fillings, Invisalign, or braces
- NightLase snoring therapy or TMJ laser therapy
- Myofunctional therapy
Read this twice: it is one $800 voucher total — not $800 for each treatment on the list.
