That payment is called a voucher. You do not pay us first and wait for money back — Choctaw Nation pays our office directly.

Only what is left over after the voucher, and after your insurance if you have it.

Three vouchers exist: $1,000, $900, and $800. Pick the one that matches your treatment.
Ask for a dental voucher and have them fax it to Broadway Dental in Poteau. Numbers are at the bottom of this page.
Once the voucher reaches us, we schedule your visit and apply the amount to your treatment.
Each one is separate. Using a crown voucher does not use up your $800 general voucher.

Insurance pays first. The voucher then goes toward what is left.
